Assistance is available to individuals and families who are in need of home healthcare
but face financial difficulties. Eligibility is determined on a case-by-case basis,
considering the client's specific situation and the resources available at the time of
application.

Types of Assistance

Depending on the individual’s needs and our available funding, clients may receive:
â—‹ Discounted Home Healthcare Services: The foundation may subsidize a
portion of the cost of home healthcare services, based on the client’s financial
situation.
â—‹ Free Home Healthcare Services: In certain cases, and based on available
resources, the foundation may provide home healthcare services free of charge.
â—‹ Medical Supplies: Assistance may also be offered in the form of medical
supplies, such as mobility aids, incontinence products, or other healthcare
essentials. Availability of these supplies will depend on the foundation's inventory
at the time of the request.

Financial Assistance Availability

The level of financial support offered may fluctuate based on the foundation's funding and resources. We cannot guarantee a fixed amount of consistency of assistance across all clients. Assistance will be allocated based on urgency, need, and available funds.

Client Responsibilities

â—‹ Provide Accurate Information: Clients are required to provide accurate and
complete information regarding their healthcare needs and financial status. False
or misleading information may result in the denial of assistance.
â—‹ Follow Healthcare Guidelines: Clients receiving assistance must adhere to all
medical guidelines and instructions provided by healthcare professionals.
â—‹ Timely Communication: Clients must notify the foundation of any changes in
their healthcare needs or financial situation that could impact their assistance.

Limitation of Liability 

The foundation aims to support clients to the best of its ability, but it is not responsible for the outcome of any healthcare services provided. The foundation does not assume
liability for any injuries, damages, or issues arising from the use of healthcare services or medical supplies.

Non-Transferability of Assistance

Any assistance provided by the foundation is non-transferable. It may not be used by
any individual other than the recipient for whom it was intended.

Right to Modify or Terminate Assistance

The foundation reserves the right to modify or terminate any assistance provided, based on changes in funding, client needs, or other relevant factors.

Confidentiality

The foundation respects the privacy and confidentiality of all clients. Personal and
medical information will be kept confidential, except where disclosure is required by law.

Dispute Resolution

Any disputes regarding the terms of assistance should be addressed through the
foundation’s grievance process, which will involve a review by the foundation’s board of directors or designated committee.

By applying for assistance from the foundation, you acknowledge and agree to the terms outlined above. We are here to support you and your family, and we are dedicated to providing as much help as possible based on your needs and our available resources.
